What types of request can I make through the online form?
You can use the online form to make requests for a replacement certificate, early certificate, name change and/or verification (to be sent directly to a board, agency or other entity). Multiple requests can be made on one form by checking all applicable options. Be sure to upload the required documents (e.g. legal papers for name changes) to avoid delays in processing your request.
When should I submit my request for verification? Verification requests should be submitted after you have been notified that you have passed the full CNT course, including both the written and practical exams.
What is the cost per request?
CCAOM charges a $15 fee per request, other than name changes which are free.
How long will it take for my request to be processed?
Due to the large volume of requests, it can take up to two weeks from the date that a request is received to complete the processing and mailing. Please complete all sections of the request form completely to avoid delays.
Can I request that my certificate or verification be expedited?
Individual certificates and verifications to colleges or state boards can be sent overnight for an additional charge of $40. The NCCAOM as well as many state boards accept verifications from us electronically, so be sure to complete the verification request accurately so that you are not charged for expedited notifications to these states.
What if I can’t remember the date and/or location of my CNT class, which is requested on the form?
Please provide an approximate date and location, which will help us locate your attendance data.
How will I know when my request has been processed?
When you complete the request form and submit payment, you will receive an email confirmation. Another email will be sent to you when your request has been processed. Please note we are only able to send verifications to the NCCAOM if you have an active application on file with them.
How can I be sure that the agency received my verification or certificate?
It is highly advisable that you check with the NCCAOM, college, agency or state board for confirmation that your verification or certificate was received. Be aware that the email received from the CCAOM is only confirmation that the request was processed.
